August 2008 Open Enrollment for
Empire BC/BS Medical
Plans
Open Enrollment for the Empire BC/BS HMO medical plans will be held between
August 1, 2008 and August 31, 2008. Open Enrollment is offered to all fulltime temporary
employees that have been employed for at least 45 days. The coverage will become
effective on September 1, 2008. The cost for these programs can be found on each
“description of benefits” listed below.
Please note that if you are a fulltime employee and have satisfied a 45 day waiting period and do not elect
coverage for either HMO medical plan by August 31st, you will not be eligible
for this benefit again until September 1, 2009. For those employees that have
not worked 45 days, once the 45 day waiting period has been satisfied, you may
elect coverage. If you do not, you will not be eligible again until next year’s
open enrollment period.
There is no open enrollment for the
dental plan. If you are a fulltime employee and your 45 day waiting period is satisfied, you may elect
coverage. If you do not elect to participate in the Rayant Dental Plan at the completion of
your waiting period, you will not be eligible to join again for 12 months from
the date you became eligible.

In order to participate in the Rayant Dental Program, you must satisfy a 45
day waiting period.
If you do not elect to
participate in the Rayant Dental Plan following your waiting period, you
will not be eligible to join again for 12 months from the date you became
eligible. |
Please note: If you are enrolling in one of the offered Empire BC/BS Medical Plans or the Rayant Dental Plan, please have your enrollment forms submitted to Frank Barone prior to the effective date of coverage.
